Job Description

Overview:

Provides general health education and HIV/STI-specific education and counseling, testing, and referral services. Offers assistance with Access to Benefits Eligibility/Entitlements, Information, Linkage and Screening for PrEP, Linkage to HIV Testing, STI Screening, Linkage to Primary Care, Linkage to Substance Use & Mental Health Services, Prevention and Supportive Counseling, Translation/Interpretation, and Treatment Adherence Counseling for HIV Care and Treatment (PrEP). Obtains blood specimens by performing venipunctures and fingersticks. Responsible for the coordination of required services, programmatic operations, and strategic planning to ensure accurate support for the program(s).

Duties and Responsibilities:

· Coordinating and implementing program goals and outcomes.

· Develop a working knowledge of existing conditions, trends, activities, and public policies affecting the community.

· Develop and maintain a service program sufficiently flexible and dynamic to meet the needs of the underserved and economically disadvantaged populations particularly those who are at higher risk for HIV/AIDS.

· Provides targeted outreach and recruitment, HIV/STI counseling, testing, and referral (e.g. blood based, Orasure®, Oraquick®, Insti®) as well as Health Education and Risk Reduction to High Risk and/or Persons Living With HIV/AIDS.

· Assists these persons by providing support in maintaining safer sex practices in order to prevent HIV/STI infection or re-infection as well as accessing CTR services.

· Evidenced Based Interventions (EBIs): behavioral, structural and locally developed evidence-based and emerging/innovative interventions and strategies for persons living with HIV, including services focused on treatment adherence and engagement/retention in medical care. Provides access to biomedical prevention services (PrEP).

· Coordinate a linkage/network system to ensure easy access to medical care, treatment, prevention and other social services.

· Provide health-coaching appointment to clients, as needed, encouraging risk-reducing and medical adherence behaviors.

· Provide effective recruitment strategies to ensure necessary client participation.

· Provides general health education (i.e. cardiovascular health, cancer prevention, diabetes prevention).

· Communicates with others (internally and externally) to provide or exchange necessary information.

· Performs quality management/assurance activities.

· Tracks collected specimens by initialing, dating, and noting times of collection; mainlining daily documentation of collections performed.

· Must be highly available and flexible to work some late nights and maintain a flexible schedule.

· Documentation of services provided in client record and required database(s).

· Other duties as described by the supervisor.

· Provides data entry and analytical support for monthly grant reporting requirements, tracks and monitors patients reached.

· Leads monthly calls with grantors, prepares data report, and reports follow-up items.

Education, Experience, Licensure/Certification and Skills/Abilities Related Requirements:

· Bachelor’s degree required.

· Two years’ experience in a professional setting.

· Knowledge of HIV/STI and related services, Homeless, and Mental Health/Substance Abuse community resources, and working with other communities and/or populations as described within the program.

· Must be comfortable and culturally competent to work with special populations including the Lesbian Gay Bi-sexual Transgender (LGBT) community.

· Ability to analyze data.

· This is a grant funded position. This position is contingent upon receipt of grant funding specifically for that purpose. All such appointments terminate upon completion of the term of the grant under which the appointment was made, or upon early termination of the grant by the funding party, or upon exhaustion of the available funding for the position provided for under that contract or grant, whichever occurs first.

Continuing Education and Training Requirements:

Participates in training required by the funding source and/or required by licensing board.
